Cooperative agreements to USA nonprofit organizations, government agencies, tribal governments, IHEs, school districts, and faith-based organizations for programs that prevent falls amongst elderly individuals and individuals with disabilities. Required registrations may take several weeks to complete.
This funding opportunity has two goals:Goal 1: Develop or expand capacity to significantly increase the number of older adults and adults with disabilities, particularly those in underserved areas/populations, who participate in evidence-based falls prevention programs to empower them to reduce their risk of falls.Goal 2: Enhance the sustainability of evidence-based falls prevention programs through the implementation of robust sustainability strategies.Applicants for this funding opportunity must propose to deliver two or more evidence-based falls prevention programs. All applicants must propose to implement at least one program that can be delivered in a remote format, i.e., by video conference or some other modality that does not involve an inperson component.
